  1. Antegrade access with short 7 Fr Sheath
  2. Wire and KMP/MPA centrally
  3. Central venogram
  4. Pullback venogram
  5. Pullback TPA infusion via KMP or other device (eg. Angiojet Power Pulse), wait appropriate amount of time for TPA to work
  6. Balloon maceration +/- PTD/Penumbra/Angiojet
  7. Fogarty outflow
  8. Retrograde access short 6 Fr sheath
  9. Wire into arterial side-glide (glidewire vs Bentson) and KMP
  10. Fogarty back through arterial anastomosis and inflow
